(١).افوض مقدمي الخدمات الطبية إعطاء العلاج وعمل الفحوصات اللازمة لتخشيص المرض او التي لها ضرورة علاجيه
(٢) أوافق علي:
(ا) نقل المريض لأي جهة علاجيه اذا كانت حالة المريض تتطلب لك
(ب) إخلاء الغرفة الغرض العزل الطبي او أسباب علاجيه أخري
(٣) أعلم بأن المستشفى يوفر خزنة لحفظ الممتلكات الشخصية(النقود،المجوهرات،النضارات،الهواتف المحمولة،الأوراق الهامة و غيرها )
وعلية فأن المستشفى غير مسؤول عن فقدان اي من ذالك مالم يتم ايداعه في الخزنه
(٤)أعلم بأن المستشفى يعتبر صرحا أكاديميا و تدريبيا، و بناء عليه فأنه ليس لدي أي مانع من مشاركة غير الاستشارين في تقديم الرعاية الصحية وذلك تحت اشراف طبيب استشاري
(٥)استلمت كتيب لائحة حقوق المرضي عند التسجيل/ التنويم وتم تزويدي بنبذة عن محتوياته من قبل الموضف المختص
(٦) اعلم ان توصية خروجي من المستشفى تتم من قبل طبيبي المعالج بما بتماشي مع حالتي الصحية لذا أوافق علي الخروج عند إتمام علاجي في المستشفى
